endometrial lymphatic vessel

An endometrial lymphatic vessel is a lymphatic vessel found within the endometrium, the inner mucosal lining of the uterus. These vessels form part of the lymphatic drainage system of the uterus and play roles in immune regulation and fluid homeostasis within the endometrium.

Endometrial lymphatic vessels drain excess interstitial fluid, transport immune cells, and help maintain the immunological environment of the endometrium. They are important in regulating immune tolerance during implantation and early pregnancy, and they play a role in the cyclical changes of the endometrium across the menstrual cycle.

Endometrial lymphatic vessels undergo cyclic remodeling in response to hormonal changes. During the proliferative phase, estrogen promotes lymphangiogenesis (formation of new lymphatics). During the secretory phase, progesterone modifies lymphatic density and permeability. These changes are coordinated with the vascular and structural remodeling of the endometrium in preparation for potential implantation.

Endometrial cancer can spread via lymphatic vessels to regional lymph nodes. Tumor cells may invade endometrial lymphatic vessels and travel to pelvic and para-aortic lymph nodes. The presence of lymphovascular space invasion (LVSI) in endometrial cancer specimens is an important prognostic factor that increases the risk of nodal metastasis and guides decisions about lymph node dissection and adjuvant therapy.

Individual endometrial lymphatic vessels are microscopic structures not directly visible on standard imaging modalities such as ultrasound or MRI. However, sentinel lymph node mapping using tracers injected into the endometrium can track lymphatic drainage pathways from the uterus, helping surgeons identify the first-draining (sentinel) lymph nodes to biopsy during minimally invasive endometrial cancer surgery.
